From a Childhood Memory to a Career Mission
Dr Fibishenko's motivation for entering implant dentistry traces back to watching his grandmother struggle with dentures. That early experience shaped a career-long focus on making full-arch implant rehabilitation more predictable, more accessible, and less daunting for patients.
Today, practising exclusively in oral implantology and dento-facial surgery, Dr Fibishenko has performed thousands of implant procedures and is recognised internationally as a key opinion leader, mentoring dentists, specialists, university professors and implant manufacturers around the world.
Solving the Problem With Temporary Teeth
One of the most significant shifts Dr Fibishenko pioneered was moving away from the traditional two-stage implant approach. Conventional protocols required patients to wear temporary teeth for months while implants healed — a period that Dr Fibishenko identified as a major source of complications.
Temporary restorations are fragile, and any cracking can disconnect individual implants from the bridge, leading to overload. Their rougher surfaces also tend to accumulate plaque and debris, contributing to inflammation and gum recession, much of which is irreversible.
To address this, Dr Fibishenko developed the All-On-4 Plus system built around what he calls "immediate final teeth" — refined, permanent restorations fitted within 24 hours of surgery rather than months later. By reducing the biomechanical burden on implants during healing, this approach became standard practice at his clinic by 2006 and has since drawn interest from colleagues around the world.
Building a Scalable, Consistent Model of Care
Delivering this standard of care consistently across multiple clinics required more than a new surgical technique — it demanded tighter integration between surgery, digital planning, prosthodontics and laboratory manufacturing. Dr Fibishenko describes the real innovation as building a treatment ecosystem capable of delivering the same quality of outcome, regardless of which clinician or location a patient visits.
Rather than opening a chain of company-owned clinics, Dr Fibishenko chose a franchise-style partnership model, working directly with experienced practitioners who already had the right infrastructure and surgical expertise. The network began in 2014 with five clinicians across Perth, Sydney and Melbourne, with growth deliberately kept measured in the early years while workflows were refined and senior clinicians were trained to eventually train others themselves.
That careful, mentorship-led approach has since scaled substantially. All-On-4 Clinic now operates through 27 All-On-4 Plus Premium Providers across Australia, each following standardised protocols, integrated patient-management software and detailed risk profiling designed to minimise variability in safety and clinical outcomes — while leaving room for patients to personalise elements like tooth shape and colour.
Treating the Cases Others Turned Away
A defining feature of the All-On-4 Plus approach is its willingness to take on complex cases. Many patients arrive at the clinic after years of frustration, having previously been told their case was too difficult or that they weren't suitable candidates for implants.
Drawing on deep, specialised experience with these challenging presentations — including severe gum disease, extensive decay, trauma and failing dental work — Dr Fibishenko and his network of senior associates have been able to offer solutions where other clinicians could not. In most cases, patients leave the clinic with permanent, fixed teeth within 24 hours of surgery, a result that is often described as life-changing for confidence, wellbeing and quality of life, not just oral function.
Mentorship at the Core
Dr Fibishenko personally trains franchise clinicians across surgical technique, restorative work and clinic operations, and this hands-on mentorship remains central to how consistency is maintained across the network. It's also, by his own account, one of the biggest drawcards for practitioners who choose to join.
Looking Beyond Australia
With the Australian network well established, Dr Fibishenko is now turning his attention overseas. Interest from dentists and specialists abroad has opened the door to international collaboration and expansion — a new chapter he says he feels ready to take on.
As he put it in the Forbes feature, the underlying goal hasn't changed: to make full-arch implant treatment safer, more predictable, and more accessible to patients wherever they are.
